Giving Card Retail Partners
If you’re a retailer who is interested in becoming a Giving Card Retail Partner, thank you! As a Retail Partner, you and your staff will help people of all ages throughout Middle Tennessee reclaim their lives from mental illness and addiction. You will be a part of making their future brighter, all while bringing customers into your own doors. In addition, your store’s name will be featured on all of the Giving Card printed materials, as well as on the Centerstone website.

- Select what seasons of the Giving Card event you want to participate in
- Fill out your company’s information
- Please include your store description for listing in our Guidebook!
- If you’ve got a website, please include your web address, as well.
- Make a copy for your records and send a copy to us:
